How Income Affects Your Medicare Premiums
You may pay higher Medicare Part B & D rates if you are considered a "high income" earner by the Social Security Administration.
What Is IRMAA And Why It Matters?
IRMAA is an acronym for "Income Related Monthly Adjusted Amount" and is an extra premium that Medicare charges you for Part B and Part D monthly premiums. View our IRMAA income charts listed below to locate your income level from what you earned two years ago. You can find Line 11 on your 1040 (this is what SSA is looking at to make this determination).
SSA always looks back at your income from two years ago to determine "this year's" IRMAA. In 2027, they're looking at 2025's filings.
You can file an SSA-44 to ask that your IRMAA be reduced or eliminated for "life-changing" events only. These are limited to:

Remember, you'll pay the "Part B" and the "Part D" IRMAA combined. This extra payment is taken directly out of your Social Security check each month. If you don't draw SS yet, you'll receive a quarterly bill to pay.

Your income may increase your monthly Part B and Part D costs.
The amounts shown are estimated monthly surcharges per person, added to the standard Part B premium and the applicable Part D plan premium. A $0.00 surcharge does not mean Medicare coverage has no premium.
Understanding the income columns
MAGI means modified adjusted gross income. For IRMAA, Social Security generally uses adjusted gross income plus tax-exempt interest from a tax return two years earlier. The projected 2027 chart therefore uses 2025 income. Married filing separately is not included and may follow different thresholds.
About the incomplete top tier
The supplied Tier 5 row included only the single-filer threshold. Its married-filing-jointly threshold and both monthly surcharges are marked “Not provided” rather than estimated. Income ranges are reproduced in whole dollars as supplied.
Does IRMAA affect what I pay for my medications?
No. If you trigger IRMAA, it will only affect your Medicare Part D premiums. The penalty is "tacked onto" the insurance policy premiums. So if you pay $10 per month for a Medicare Part D plan, and your IRMAA is a $20 increase, then your total monthly premiums will be $30.
